School Lunch Program

Visit www.lunchapp.com to apply for CCS's free and reduced lunch program.  The website offers a convenient and secure way to sign up for the program.

 

 

Medication Forms

The following forms should be filled out completely and returned to Sabrina Calderon at Challenge Charter School.

Non-Prescription Medication Form

Authorizes your child to receive certain non-prescription medications from CCS.

Prescription Medication Form
Must be completed and signed for all prescription medication to be distributed at school.

 

Self-Carry Medication Form
Must be completed and signed for a student to be permitted to carry or keep medication in his or her possession.

 

Transportation Forms

The following forms should be filled out completely and returned to the front office at Challenge Charter School.

New Student Bus Transportation Form
New students only-Complete this form to request pick-up or drop-off by CCS bus.

Bus Transportation Form
Complete this form to request pick-up or drop-off by CCS bus.

Day Care Transportation Form
Complete if your child is to be picked up after school by any day care provider.

 

Change of Information Forms

The following form should be filled out completely and returned to the office at Challenge Charter School.

Change of Information Form
Use this form to update your contact information.  It is very important that the school has updated contact information on file.

Photo Release Forms

The following forms should be filled out completely and returned to the front office at Challenge Charter School.

Photo Release Form
Use this form to grant permission for your child to be photographed or filmed at CCS.

Single Event Photo Release Form
Use this form to grant permission for your child to be photographed or filmed at CCS for a single event.

Pre-Registration Form

The following form should be filled out completely and returned by mail or fax to the office at Challenge Charter School.
